This is the contact point for requests to disclose sender information under the Provider Liability Limitation Act. You will need to submit a written disclosure request, evidence establishing the rights infringement, and identity verification documents (or an attorney's ID). We accept requests even if these documents are not ready at the time of submission, but we may ask you to provide them afterward. Without the documents, we cannot make a disclosure decision.
